It is present in about 40% of all ow back pain patients that had spinal fusion. What is the SI joint? This is the spine. This is the sacrum which is the lower part of the spine. The pelvis is connected to the spine through the SI joints. Why is this connection important? Because the SI joint transfers the force and the load from the spine to the legs. The Si joint has very little movement; you don’t want a lot of movement. The Si joint is limited to less than four degrees pf rotation and bout 1.6 mm of translation, so the SI joint has very little movement. The cause of the pain in the SI joint cannot be due to this little movement. Some people think it is neuroplasticity when the nervous system is very sensitive to any movement and the ligaments around the SI joint is full of nerve fibers. Neuroplasticity is rewiring of the nervous system in response to injury to function differently than before. It is a maladaptive reorganization of the nervous system to a painful site which can be resolved with treatment. In general, the cause of the pain is usually difficult to determine. SI joint is protected by strong ligaments that prevent shear forces, and it is surrounded by strong muscles for stabilization during movement. The causes of SI joint problems are several. Pain from different sources overlap. The SI joint, the lumbar spine, and the hip share the same pain. The pain may be associated with one of these areas or all of these areas. In fact, more than one source of pain may exist. So the diagnosis of SI joint dysfunction can be challenging and the patient will go through a battery of steps including injection with greater than 75% in reduction of pain. The cause of the pain is usually idiopathic and multiple risk factors may exit. Usually, the idiopathic SI joint dysfunction will not show up on the x-ray, MRI, or CT. In fact, up to 25% of asymptomatic patients over 50 years old will have abnormal SI joints on x-rays.Risk factors / causes of SI joint pain? The important risk factor is previous lumbar fusion, especially when there is more than three levels involved, similar to adjacent segment disease. So if the spine is fused, then maybe the SI joint will move more and become painful. Pregnancy can cause SI joint pain. Previous trauma to the pelvis can cause SI joint pain. 80% of patients have a history of a specific trauma, usually a twisting injury. And Si joint pain can occur from getting bone graft from the pelvis and violating the SI joint. Always obtain a detailed history of trauma, infection, inflammatory disease such as Ankylosing Spondylitis, Reiter’s Syndrome, Psoriasis, check for spinal fusion, Scoliosis and leg length discrepancy. When you examine the patient, the pain is usually not in the midline, the pain is usually to the side. The pain will be below L5 level. How do you now that? Look at that pelvis. This is the level of the iliac crest and this line corresponds to the L4 level. The pain will be below the L5 level. Only 4% of patients had pain above L5. Usually, the SI joint pain is just inferior to the posterior superior iliac spine (PSIS). Treatment of the SI joint is usually non-operative treatment including anti-inflammatory medication, physical therapy and pelvic belt. Try to do at least four weeks of non-operative treatment before doing the injection. If you suspect the SI joint to be the cause or the main pain generator, then you need to identify the area of pain exactly, and that will be done by a diagnostic injection to confirm the diagnosis. The injection might give 60% success rate in pain relief at six months. If you have more than 75% reduction of the SI joint following a single injection, that means that the source of pain is the SI joint. You have to do it under fluoroscopy or ultrasound. If you do it blind, only 22% of the needles will be in the SI joint. Injection can be diagnostic and therapeutic, but do not give more than three injections in six months or four injections within one year. The radiofrequency ablation targets the lateral branches of the sacral nerve roots, but it is unable to denervate the anterior neural structures of the Si joint. Fusion is minimally invasive. You have got to have confirmation that the pain comes from the SI joint, the SI joint is the primary pain generator, and you have to have poor response from non-operative treatment. The procedure can be done minimally invasive and as an outpatient. Result has better outcome compared to conservative treatment. Arthrodesis is good option for a patient with confirmed sacroiliac joint dysfunction and poo response to non-operative treatment. SI dysfunction is a disabling problem.

We are going to group all of the causes of ankle pain into 4 groups: Anterior ankle pain (in front of the ankle) Anterior lateral impingement: This is painful limitation of full range of motion of the ankle due to soft tissue thickening or tibial bony spur impinging on the talus, limiting dorsiflexion of the ankle. Arthritis of the ankle: Arthritis of the ankle can occur due to injury or due to inflammation. It can be diagnosed by examination or by an x-ray. Osteochondritis dissecans of the talus: A little chip fracture inside the ankle that causes pain, swelling, and stiffness. Medial ankle pain (inside of the ankle) Tarsal tunnel syndrome: Compression on the posterior tibial nerve produces symptoms of pain and numbness on the medial (inside) side of the ankle. When conservative treatment fails, surgical treatment of the tarsal tunnel release may be needed. Posterior tibial tendon tendonitis or rupture: Posterior tibial tendon problems can occur from overuse activities, from degeneration, or trauma. The posterior tibial tendon is one of the major supporting structures of the foot. The tendon helps to keep the arch of the foot in its normal position. When there is insufficiency or rupture of this tendon, the arch begins to sag, and a flatfoot deformity can occur associated with a tight Achilles tendon. The posterior tibial tendon rupture occurs in the hypovascular area, which is distal to the medial malleolus. The patient will have painful swelling on the posteriomedial aspect of the ankle. The patient will be unable to perform a single heel toe raise, too many toes sign, or flatfoot or fixed deformity of the hindfoot. Rupture of this tendon could be missed! Flexor hallucis longus tendonitis: Pain, swelling, and weakness posterior to the medial malleolus. Dorsiflexion of the big toe may be reduced when the ankle is placed in dorsiflexion. Triggering and pain along the tendon sheath may also occur with toe flexion. Posterior ankle pain (behind the ankle) Posterior ankle impingement (os trigonum): Os trigonum is a nonunited piece of accessory bone seen posterior to the talus. Tenderness in the posterior lateral aspect of the ankle posterior to the peroneal tendons, especially with passive plantarflexion of the ankle, may be seen in association with Flexor hallucis longus (FHL) tenosynovitis. Achilles tendonitis: Irritation and inflammation occurs due to overuse. Pain, swelling, and tears within the tendon. It is usually treated with therapy and eccentric exercises. It is rarely treated with surgery. Achilles tendon rupture: The Achilles tendon can become prone to rupture with age, lack of use, or by aggressive exercises. Rupture of the Achilles tendon is diagnosed by the Thompson test or by an MRI. Treatment may be conservative without surgery by doing early functional rehab. Or it can be treated with surgery by approximation of the torn ends of the tendon. The risk of surgery is infection and skin/wound complications. The risk of conservative treatment is a high rate of rerupture of the tendon. Lateral ankle pain (outside of the ankle) The cause of ankle pain on the outer side of the ankle may be a sprained ankle, but diagnosis of a sprained ankle may be the wrong diagnosis. There are other structures in this area that can mimic an ankle sprain, but it is not an ankle sprain. Ankle sprain: When the pain is anterior and around the fibula, it can be attributed to ligament sprain. Ankle sprain can be high ankle sprain (rare) or a low ankle sprain (common). Both types of ankle sprains are seen in this diaphragm. High ankle sprain: High ankle sprain is a syndesmotic injury and requires surgery. The high ankle sprain is a serious injury because it injures the ligament that connects the tibia to the fibula at the ankle joint. The diagnosis is usually done by the use of external rotation stress views or by CT scan. Low ankle sprain: A low ankle sprain is not a serious injury and it does not require surgery. Low ankle sprain is collateral ligament injury. With an ankle sprain, although the condition is painful, the patient will be able to walk. When there is a fracture of the ankle, the patient will be unable to walk due to severe pain. The treatment of ankle sprains will include immobilization, ice therapy, physical therapy, and rarely surgery. Diagnosis of a sprained ankle may be the wrong diagnosis. So, let us see what else can cause pain on the outside of the ankle. Peroneal tendon subluxation: Occurs due to rupture of the superior peroneal retinaculum. Rupture of the peroneus longus tendon: The os peroneum displaces proximally. Peroneal tendinitis Anterior process of the calcaneus fracture Lateral process of the talus fracture

Fix Rhomboid Pain

Are you dealing with a nagging discomfort in between your shoulder blade and your spine? No matter how many stretches you do, or how much time you spend foam rolling it, does it still persist? Pain in this area often gets labeled as a rhomboid issue because it is a relatively superficial muscle that you can appreciate from any anatomy chart. However, how to fix rhomboid pain sometimes may actually have little to do with the rhomboids. In this video, you'll learn the ultimate secret of how to fix rhomboid pain by taking a look at a very specific movement...thoracic rotation! Rhomboid pain is labeled as pain in between your shoulder blade and your spine (interscapular pain) that can even refer up to the base of your neck or down the middle of your back. It is often described as a nagging dull ache or pain, pressure, knifelike, pulling, or even a burning sensation (is that really rhomboid pain? Food for thought). You'll often find people dealing with discomfort in this area wanting to fidget around in efforts to feel better - moving their head and neck around trying to stretch, rounding their shoulders, squeezing their shoulder blades back, or twisting their upper back, you name it. The occasional soreness in this region is nothing to be concerned about, especially if you did a hard upper back workout or carried a backpack for a long time. However, dealing with constant discomfort or pressure in this area can become a real nuisance. Just because you're dealing with discomfort in between your shoulder blade and your spine doesn't mean we have to point the finger at the rhomboids every time! To fix rhomboid pain, we need to take a look at and consider all of the other anatomical structures in the area. If there was only one movement assessment I had to pick when it comes to evaluating interscapular pain, I'm going to look at thoracic rotation mobility. Poor thoracic rotation mobility can wreak havoc on the body and can definitely refer pain and discomfort to the rhomboid region. Poor thoracic rotation mobility is often coupled with poor scapular mobility/stability, which only contributes to the issue more! I have found that simply improving thoracic rotation mobility can improve interscapular discomfort. How to Fix “Low Back” Pain (INSTANTLY!)

The irony is, a lot of times what we feel is rooted in the lower back is actually caused by muscles not in the back at all. In this video, I’m going to show you how the glute medius could be the real cause of your back pain and a quick exercise you can do to relieve your discomfort instantly. Once gone, I’m also going to show you a few additional exercises you can do to make sure your low back pain never returns. The gluteus medius is sandwiched between the glute maximus and minimus and lies in and around the hip area. The role of the muscle is to abduct your hip or lift your leg out to the side in either standing or side lying and to keep your pelvis level whenever you take a step. Prolonged sitting during the day as well as an unequal weight distribution when standing are two of the most common reasons for this muscle to get weak and imbalanced. You can quickly test to see if you have the makings of a weak glute medius by standing with your feet shoulder width apart and lifting one foot off the ground. First take note whether or not your opposite hip drops significantly. If it does, this would indicate a glute weakness on that side. You would repeat with the opposite foot as well. When you do this you would also want to see if you had to dramatically shift your weight to one side just to lift that foot off the ground. If you do, this would indicate that you have an unequal weight distribution when standing and it would be especially troublesome when squatting. To fix this quickly, you will want to lay down on the ground with your affected side on top. If your right lower back was bothering you then you would want to lie on your left hip. From here, take your thumb and place it on the area most sore. You should feel that this is going to happen just to the outside of the bony prominence of your pelvis. From here, push to hold back the trigger point and start flossing your leg down and in front of you and then back and up. Your hip should be extended and then lifted into abduction towards the ceiling (being sure to point the toes down to keep the glute medius in focus). Do this about 10 times until you feel the tension in the muscle release. Now, you can burn out the spasm in the trigger point by getting into the fully contracted position of the glute medius muscle and holding as long as you can. Generally, because this muscle is often very weak, this may not be any longer than 30 seconds to a minute. Once you cannot hold it any more you will stand up and you should notice an immediate relief of the pain on that side. This is the quick but not permanent fix for this problem. Since the underlying cause is weakness in the gluteus medius muscle you will want to back this up with some exercises for your low back that you can do a few times a week. I show you three options for this. The first is the hip bump against the wall. The second is called the sack swinger, and can be done with a dog leash if you don’t have a formal dip belt. The last is actually doing the same movement that you did for the treatment, but this time as a strengthening exercise for your low back. Whatever you do, just be sure you are consistent. You will also see that as you relieve the tension in this muscle that your squat performance improves as well. Your depth should be increased and any low back pain that you felt by doing the exercise should be gone since you will now be able to do them with equal force through each leg. Have you tried stretching or rolling this area with a lacrosse ball or foam roller only to get temporary relief? Considering the location of the rhomboids, many individuals are led to believe that their discomfort originates from a strain or trigger point associated with these muscles. However, the rhomboids are rarely the culprit so in order to find a long-term solution, we should take a step back and consider a more holistic approach. It’s Not Your Rhomboids (0:45) So if it’s not your rhomboids, what is it and why are you experiencing discomfort in this area? A landmark study by Dr. Ralph Cloward in 1959 applied stimuli to different aspects of the cervical intervertebral discs on conscious patients and had them report where they experienced discomfort. As you can see from the images, a common location for pain was in this interscapular area. Other studies have replicated this finding as it relates to the discs such as Slipman et al in 2005. And Dwyer et al 1976 is well known for stimulating the zygapophyseal joints in asymptomatic subjects and mapping their reported pain. The last consideration, and the focal point of this video, relates to irritation of the lower cervical nerves. You might generally associate this irritation with neck pain and pain down the arm in a nice dermatomal distribution, but Murphy et al 2009 found that occurs in less than ⅔ of cases and around 50% of people report pain in that rhomboid area. In fact, Tanaka et al. 2006 reported “scapular region pain is generally the initial symptom in radiculopathy and can persist alone before the arm or finger symptoms develop.” And in some cases, it might be the only symptom that ever really occurs. So although you might be experiencing discomfort in the rhomboids, that sensation is likely more of a secondary response. Also, I understand that issues with the neck or a nerve sound scary to a lot of individuals, but it doesn’t mean anything is damaged, pinched, etc. I just like to think of it as something being sensitive. Neck pain causes and treatment Patients can complain of neck pain from a variety of reasons and in this video I am going to try to outline the most common causes of neck pain and how to treat it. The first thing you want to identify is if the patient has neck pain or shoulder pain. Shoulder pain and neck pain problems overlap. You need to know if the origin of the pain is the cervical spine or if the shoulder itself is the origin of pain. Does the patient symptoms improved by shoulder abduction and placing the hand over the head, then the pain has the cervical spine origin. Shoulder abduction test Lifting the arm above the head usually relieves the symptoms because it decreases tension on the affected nerve. Is the pain more when you move the shoulder? Then the shoulder itself is the source of the pain. An important cause of neck pain is disc herniation. The patient will complain of numbness, paresthesia and radiculopathy, which is shooting pain that radiates from the neck to the arm. The patient will have unilateral arm pain, numbness and tingling in a specific dermatome in the hand. As you see here in this diagram. There will be weakness in a specific muscle group. And cervical radiculopathy, the pain is sudden and goes to the arm. Radiculopathy occurs due to compression of the nerve root either by a disc herniation or by arthritis that narrows the foreman. Cervical disc herniation occurs most frequently at the level of C6-C7 and it will affect the C7 nerve root. It can also occur at C5-C6 and in this case, it will affect the C6 nerve root. How to you test for cervical spine disc herniation? Neurological exam of the patient will include: Motor testing Sensory testing Reflex testing This is how you test the motor strength of the nerve root from C5-T1. This is how you test the sensory as you see her in this diagram. This is how you test the reflex as you can see in this diagram. Spurling test for cervical spine radiculopathy What is the Spurling test? The Spurling test is considered to be positive with neck extension and rotation towards the painful side reproduces symptoms in the ipsilateral arm. The natural history of cervical radiculopathy is favorable with resolution of the symptoms in most cases. And cervical radiculopathy, the patient will have 70 to 80% successful outcome after 2 to 3 months with conservative treatment. How do you really treat cervical disc herniation? We are going to start nonsurgical treatment first. Use anti-inflammatory medication, isometric exercises, physical therapy, and muscle relaxers. Get an MRI if symptoms do not improve after 6 weeks of conservative treatment. The MRI result should be correlated with the clinical symptoms. False positive rate of the MRI is high. . In asymptomatic patients older than 40 years old, you will find that 57% of these patients will have at least 1 degenerative disc on the MRI. The MRI may show you a problem with the disc in patients you are not complaining of any neck pain. Therefore, use a result of the MRI wisely and do not scare the patient, because what you find on the MRI could be a normal process. Persistent disabling pain for 6 to 12 weeks despite nonoperative treatment can be an indication for surgery. The surgery is usually anterior and it involves decompression and fusion of the involved disc space. Asked the patient if the patient has gait disturbance (unstable gait when walking, wide-based gait) or hand clumsiness, may indicate cervical myelopathy due to compression of the spinal cord (serious condition). The patient may have decreased manual dexterity and difficulty manipulating fine objects such as buttoning and on buttoning shirts. The patient may have occipital headache and a sense of discomfort in the neck. The patient may not have severe neck pain. In general, and cervical myelopathy the onset of pain is insidious, gradual, poorly characterized, and localized. Cervical myelopathy is a slow progressive process with stepwise progression and deterioration over time. In cervical myelopathy, the MRI will show compression of the spinal cord. Cervical myelopathy you will find upper motor neuron signs including a positive Hoffmann sign. This is how you will see a positive Hoffmann sign. What are other upper motor neuron signs? You will find the patient will have hyperreflexia, positive Babinski test and clonus test. Early recognition and early surgery is important for a good outcome. The severity of the symptoms and early treatment is most important to the outcome. Surgery is done for any functional impairment of the gait or the hands. Usually the treatment in this situation is surgery back decompression and fusion.e condition of neck pain and cervical disc herniation

It is the characteristic presentation of some sort of irritation to the sciatic nerve. WHAT CAUSES SCIATICA These sensations of numbness, tingling, or pain are the result of an irritation to the sciatic nerve. Whether it’s a disc in your back, your vertebrae, or a muscle - anything that impinges and puts pressure on the sciatic nerve can result in sciatica nerve symptoms. BEST EXERCISE TO TREAT SCIATICA Now depending on the exact cause of your sciatica, the treatment exercise progression looks a little different. For example there are certain exercises I would give to a patient with sciatica from a bulged disc that I absolutely would NOT recommend for someone with sciatica from stenosis or degenerative disc disease (it’s crucial that you check with a local healthcare provider for exercises specific to your unique diagnosis). While these various pathologies are all treated a little differently, today I wanted to demonstrate for you the one exercise I give the majority of my patients regardless of the cause of their sciatic nerve pain. THE BEST EXERCISE FOR SCIATICA NERVE PAIN Think of your nerve as a “string” (or “floss) from your back to your big toe. Somewhere along the course of this nerve, something is “pinching” the nerve and causing pain (or numbness or tingling) sensations. What we need to do is “stretch” or “floss” this nerve back and forth to free it up from this “pinching” and decrease the painful nerve signals. This is best accomplished with “nerve glides” or “nerve flossing”.
